HUMAN SERVICES COUNSELOR III
This is general work as a counselor for all counseling occupations not listed separately. Responsible for the implementation and compliance with the rule officially known as Chapter 64D-4, Eligibility Requirements for HIVIAIDS Patient Care Programs, Florida Administrative Code. Specifically, this position is responsible for assessing and determining patient eligibility for the DOH HIV Patient Care CORE ELIGIBILITY (Chapter 64D-4, Florida Administrative Code) and then Program Qualifications for the AIDS Drug Assistance Program as established by the Florida Department of Health. This position reports to the Operations & Management Consultant Manager of the ADAP Program at the Florida Dpt. of Health in Miami-Dade County.
Reviews all applications received from the pre-screening sites at the community case management agencies and from applicants who apply directly to the program. Incumbent makes eligibility determinations under the DOH HIV Patient Care Core Eligibility rule, as defined and determined in Chapter 64D-4, Florida Administrative Code. Specifically, incumbent is able to make an eligibility determination for DOH HIV Patient Care Programs in Florida for patients applying to the AlDS Drug Assistance Program (ADAP). Complies with the established procedures to issue DOH HIV Patient Care Core Eligibility Notices of Eligibility, Ineligibility and Request for Exceptions, as presented in the Core Eligibility Procedures manual. Reviews clients' HIV patient Care Core Eligibility every six months in order to be qualified to continue receiving services. Keeps track of all information by entering confidential patient information in the local Core Eligibility database (ACCESS) and the statewide database (CAREWare) and producing the proper documentation as required by the State.
Enrolls eligible patients who are applying for services from the AIDS Drug Assistance Program by ensuring that they comply with the DOH established Program Qualifications in order to receive services from the program. Coordinates with case manager at authorized pre-screening sites in order to ensure that proper documentation is kept in the patient file, readily available for audits conducted by the State of Florida Auditor General Office, Florida Department of Health or any other authorized entity as allowed by Federal and State laws. Enters patient information in the DOH Statewide Electronic ADAP database provide enterprise andissues official documents confirming patients' qualification to receive services from the program; coordinates and informs pharmacy about proper decision in order to ensure pharmacy compliance by dispensing medications to the eligible and qualified clients only. Assists clients with treatment compliance and adherence to treatment issues through different activities as part of the treatment adherence and compliance program efforts. Addresses program compliance requirements with the applicant and assists them to remain inthe program through different activities as part of the treatment adherence and compliance program efforts.
Provides services by assisting insured clients with the AIDS Premium Plus Program including determinations of need for assistance with the co-payments, deductibles and monthly premiums; confirms that client has a current health insurance policy and procures the insurance card, explanation of benefits, formulary and provider information.
Coordination of access to medications through the referral of the Premium Plus clients to pharmacies under thePharmacy Benefit Manager, currently CVS/Caremark; issues the CVS/Caremark secondary insurance cards to clients and explains the process to access the services and use the card. Provides assistance to Premium Plus clients when pharmacy services are rejected by the PBM provider; coordinates with selected pharmacy staff, insurance company and prescription drug provider, as needed. Coordination of services with specialty pharmacy providing prescription drug services to the Premium Plus client, to access HIV medications through the contracted Pharmacy Benefit Manager.
Assists clients with screening process, required new documentation and referrals to apply for the Healthcare Insurance Marketplace as determined by the state; coordination of services and referrals with the clients medical case managers and ACA MP Navigators, Certified Application Counselors for the application for one of the insurance plans selected by DOH for ADAP-Miami clients.
Coordination of services with Ryan White Part A for the provision of wrap around services including but not limited to copays for the necessary outpatient medical care, laboratory services as required by the ADAP program, as well as their access to non-HIV medications not available through the ADAP formulary.
KNOWLEDGE, SKILLS AND ABILITIES:
- Knowledge of HIV/AIDS, basic AIDS treatments and adherence to treatment
- Knowledge of interviewing techniques
- Knowledge of computers and computer software. Microsoft: Excel, Word, PowerPoint, Access, Outlook and other Pharmacy related software.
- Ability to plan, organize and coordinate work assignments
- Ability to collect and analyze health related information
- Ability to establish and maintain effective working relationship with community based organizations providing services to HIV/AIDS patients
- Skills in planning, organizing, coordinating, implementing, and educating HIV/AIDS patients on treatment and adherence education issues.
- Bachelor's Degree from an accredited college or universityand two years of work experience in Health, Health Administration, social work or related fields.

Incumbent may be required to work before, during, and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limit to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
